INSURANCE POLICIES
We realize how important insurance benefits are to you. Please
realize that insurance benefits are a supplement only. They do
not pay for everything. We ask that you carefully review your
policy and contact your insurance carrier so that you are aware
of your benefits and frequency limitations.
Please realize that dental insurance is a contract between you
and your insurance company. We will assist you with filing
claims promptly and accurately and to help you maximize your
benefits. Please be aware that most insurance have annual
deductibles and a yearly maximum. Anything over that amount is
your responsibility. Please provide us with a copy of your
insurance card and benefits book at your initial appointment.
Because insurance policies vary greatly, we can only estimate
your coverage in good faith but cannot guarantee coverage due to
the complexities of insurance contracts. Your estimated patient
portion must be paid at the time of service.
As a service to our patients, we will bill insurance companies
for services and allow them 45 days to render payment. After
that time, you are responsible for the entire balance,
paid-in-full. Please contact your insurance company directly to
find out why payment has been delayed.
We are looking forward to taking
care of your Dental needs! |
